Seeking passionate and organised individuals to play a pivotal role in coordinating and delivering outstanding clinical care within our dynamic Specialist Community Mental Health Service.
Responsibilities: Deliver comprehensive clinical care coordination: Facilitate communication and collaboration between patients, clinicians, and community stakeholders Develop diverse professional portfolios: Gain experience in depot and clozapine clinics, supporting consumer fitness groups, and facilitating therapeutic interventions.
Conduct thorough assessments: Utilise your expertise to perform mental health and psychosocial assessments, risk assessments, and develop personalised treatment plans.
Support team leadership: Collaborate with the Team Manager to contribute to a cohesive and efficient team environment.
Contribute to quality improvement: Engage in clinical auditing, research, and evaluation initiatives to continuously improve patient care outcomes and service delivery.

Generous relocation assistance available to interstate and overseas candidates About you: You are a Registered Nurse with the Nursing and Midwifery Board of Australia, holding a current practicing certificate; You are Enrolled in an approved mental health course or qualification in mental health practice; You have demonstrated clinical expertise and experience in mental health services; You have strong communication, collaboration, and leadership skills; and A commitment to continuous professional development and evidence-based practice.
