FSH also delivers specialised services such as heart and lung transplantations and hyperbaric medicine.Job DescriptionThe role of the Clinical Nurse Specialist is to work autonomously and in partnership with the multidisciplinary Cardiology Service to provide continuity and excellence in care for patients with suspected or diagnosed cardiology conditions. The CNS is primarily responsible for navigating and coordinating referrals, clinics and waitlists to improve the patients journey and experience. The CNS supports delivery of quality patient care through developing and implementing a coordinated, evidenced based, seamless, patient focused approach to providing nursing care. This includes development of standards of practice, implementing changes to clinical practice, educating patients and staff, and initiating research and quality improvement activities.
