Rockingham Peel Group is part of the South Metropolitan Health Service, one of the fastest growing health services in Western Australia and comprises two hospital campuses, Rockingham General Hospital and Murray District Hospital, in addition to community and mental health services across Peel, Rockingham and Kwinana.
Currently we are seeking a Clinical Nurse Specialist in the Aged Care Transition Liaison Nurse team.
You will work with the aged care service at Rockingham General Hospital, which is within the Rockingham Peel catchment area and with primary health care providers to identify, assess and support patients at risk of unsuccessful discharge from hospital.
You will support and monitor the transition of care from hospital to primary health care providers while also working collaboratively with geriatricians, general practitioners, nursing colleagues, and allied health staff to coordinate and support effective discharge activities, develop long term care plans, and educate and enable patients to manage their health and social care.
