The service offers a patient-centred approach, focusing on comprehensive assessment, care planning, and coordination to ensure the timely and appropriate discharge of older patients. This position services multiple sites across the Tablelands sector and involves managing patients with challenging behaviours.The role requires effective time management to prioritise workloads, ensuring the older person, their carer/family, and the healthcare facility's needs are met, including providing the first face-to-face AARCS intervention within one working day of referral. Clinicians advocate for older patients and their families while balancing conflicting priorities and must communicate effectively with families and service providers to achieve the best outcomes.
