Provide clinical and case management support to the older clients of the Wide Bay Mental Health & Specialised Services to achieve the best possible clinical outcomes for this complex group.
Liaise with all relevant clinical and non-clinical stakeholders within the older persons environment as well as non-clinical support workers in the community to coordinate a holistic approach to recovery and wellbeing.To provide expert clinical and patient-centred care within Community Mental Health by supporting the facilitation and provision of quality and cost-effective nursing services which are consistent with contemporary standards and evidence-informed practice.Employment Type: Permanent Full Time (76 hours p.f.)
