This role will see you having accountability for both the Ambulatory Liaison Nurse Service and the Silverchain Hospital Liaison Nurse Service (SCLN) teams. The service provides 7/24-hour service covering WA but also into other states after hours.
The ALN team support the clinical services in coordination of referrals and provide referrers information on services available /eligibility and follow up as appropriate. The SCLN team support the Perth Metropolitan Hospital Network for incoming clients to Silverchain and follow up current clients who are inpatients on a need's basis.
The Clinical Nurse Manager role will have responsibility for the day-to-day coordination of these teams including recruitment, rostering and budget management. Nurturing of Internal and external relationships with key stakeholders is a key component of the role.
Key ResponsibilitiesThis is an excellent opportunity for a qualified Senior Registered Nurse with proven leadership experience and a strong commitment to ensuring positive outcomes for Silver Chain clients in their home.
