The YNA Community team is growing and we are always keen to hear from experienced Registered Nurses interested in Clinical Nurse roles in our team. We have opportunities for experienced, passionate & motivated Registered Nurse leaders with demonstrated experience to support the YNA clients with high health and complex needs living in the community. This is ideally a full-time role, based from Adelaide.
The purpose of this position is to: Case manage a portfolio of direct care paediatric clients.Ensure client-centred, individualised care, positive client outcomes & enhance the care of clients in the community.Coordinate & support the provision of comprehensive multi-disciplinary care involving assessment, prevention, referral & health promotion.Be a valued part of the community and clinical nurse team.Actively attract & retain health & support staff for the YNA team.Establish & maintain professional relationships with staff, clients & their support network.About You: Successful applicants will demonstrate:
