Clinical Nurse (Discharge Planner) (Maryborough) An exciting opportunity is currently available for an experienced Clinical Nurse (Discharge Planner) to join the Integrated Care Team at the Maryborough Hospital within the Wide Bay Hospital and Health Service. The Wide Bay area is arguably one of the most beautiful regions of Queensland. The Health Service stretches from Agnes Water in the North to Maryborough in the South and provides acute care services to a population of over 225,000 residents including several rural communities.

Eligible Applicants who move to Queensland from an interstate or international jurisdiction to take up permanent or temporary greater than 12 months employment may receive up to $20,000 incentive payments under the Queensland Health Workforce Attraction Scheme.
Your opportunity: To provide expert clinical and patient-centred care within Integrated Care Services by supporting the facilitation and provision of quality and cost-effective nursing services which are consistent with contemporary standards and evidence-informed practice. The Clinical Nurse (Discharge Planner) supports the Nurse Unit Manager and other members of the Integrated Care Services team with the timely transitions of care through the patient journey and the provision of clinical leadership, advice and advocacy to a multidisciplinary team. Our offer: You will receive a competitive remuneration package value up to $65,950 p.a., comprising salary between $54.45 - $58.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Part Time, 38 hours p.f.)
