The Diabetes Service delivers nurse-led diabetes education clinics and contributes to a broader diabetes multidisciplinary medical and allied health team to provide expert, quality, accountable care and education in diabetes to patients across the care continuum and in accordance with prescribed professional and ethical standards and evidence-based practice.Temporary Full-time and Part-Time opportunities.The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners, and other agencies (including non-Government agencies).
This role provides the delivery of clinical care and support specific to the Diabetes Service within an outpatient and inpatient setting.About You
Our ideal candidate will be someone who can demonstrate the following:Demonstrated advanced clinical knowledge in patient assessment, education, and management for patients (adults & children) living with diabetes, contemporary insulin systems (MDI and CSII), and skills in flash and CGMS initiation and data interpretation delivering expert nursing care in complex situations while providing support and direction to other team members.Demonstrated ability to build and maintain strong relationships and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.Demonstrates a strong team commitment through ability to provide clinical education and support at an advanced level.Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives, and visitors and maintain a positive workplace culture through well-developed consultation, liaison, negotiation, and conflict resolution skills.Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.Pro-actively manages own continuous learning and development.West Moreton Health is one of the most rapidly evolving health services in the southeast corner, with a community growth expected to almost double to more than 588,000 by 2036.
