Seeking a Clinical Nurse Consultant - Transition Care Program to provide expert clinical advice and clinical leadership within a multidisciplinary team in the transition care setting, enabling the effective assessment, case management, care coordination and implementation of planned, goal-oriented transition care to older people in the community and/or residential aged care setting.
The Role: Initiate and maintain internal and external educational programs to facilitate the provision of care for older people in collaboration with the NUM and the multidisciplinary team.Provide an advanced level of professional clinical expertise and management to ensure planned, goal-oriented transition care to older people.Consult with the health care team and contribute to recommendations in relation to planning, implementing, and evaluating clients' care to ensure best practice.Champion a culture that upholds the National Principles for Child Safe Organisations and ensures the safety of all vulnerable people.Advocate for the safety, wellbeing, and empowerment of the frail elderly patients in our care.Details of appointment: Permanent part-time, day worker position, working 20 hours per fortnight, commencing as soon as we can onboard you.
