You will be working in the community with older adults experiencing mental health issues who may be living in their own homes, with family, or in RACF.
You will be providing recovery-oriented, person-centred biopsychosocial care planning and interventions reflected in all aspects of your practice and service delivery.
You will have previous experience working with older adults in a health or community setting, including experience of assessment, care planning and interventions which are appropriate for and effective with older adults experiencing mental ill health.
Previous experience in a care coordination role will be of benefit.
Are you an experienced and skilled nurse with relevant post graduate qualifications; That is experienced, patient and empathetic when working with older adults, their families and caregivers?

The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership facilitates evidence based practice.
The CNC will promote a service which fosters clinical excellence, research and professional leadership.
The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes.
The CNC is responsible for providing services which facilitate effective management of the care in the OPMH Speciality.
