CANAS is a multidisciplinary team that focuses on evidence-based practice, continuous quality improvement, and risk management, including information-based decision making as mechanisms for improving the quality, standards, and safety of the health service provided.
The principle objective of CANAS is the substitution and avoidance of hospital presentation, admission, and facilitation of early discharge to enable restoration of functional capacity.
About the role:An opportunity exists within our Complex Care Co-ordination (CCC) team.
The Complex Care Co-ordination team supports clients over 18 years of age to manage complex or unstable health conditions in the community.
The clinician will utilise specialised nursing competency in the coordination and delivery of direct/indirect advanced client-centred care to high-risk, complex client groups with chronic disease, to improve the client journey and flow through CQ (Central Queensland) Health services to minimise hospital presentations and ensure appropriate transition for ongoing client management in the community.
Complex Care Clinical Nurses provide home visits within CANAS service area: Rockhampton, Yeppoon (including Emu Park) and Mount Morgan.
About you:Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).Possess advanced clinical knowledge and skills in gerontology, with a specific focus in geriatric syndromes and chronic disease.Effective clinical leadership that ensures consistent delivery of high standards of evidence-based nursing care and a commitment to continual quality improvement.Demonstrated commitment to participating in professional development activities.What we offer:Total remuneration value of up to $65,949 is comprised of:
