To provide expert clinical knowledge and care specific to discharge planning, admission avoidance and risk assessment for patients with complex discharge planning issues.
The purpose of this role is to provide expert clinical knowledge and care specific to discharge planning, admission avoidance and risk assessment for patients with complex discharge planning issues. This role will support the optimal function of the unit through role modelling and application of the principles of contemporary evidence-based patient-focused care, education, leadership and portfolio management specific to complex discharge planning and case coordination. In addition, this role will provide support and direction to the Registered Nurse and other non-registered nursing personnel in more complex situations.
