Our workplace is fast paced, challenging and supportive, where every employee plays a critical role in our success. The Continuing Care Team at the Mackay Community Health Centre is seeking applications for a Clinical Nurse (Community) to join their team on a permanent basis.
Your opportunity The successful post holder will function as a Mental Health Clinical Nurse as part of a multidisciplinary team responsible for coordinating and delivering case management and care coordination activities. The Clinical Nurse will manage a case load that supports individuals with serious mental illness to provide specialist assessment, crisis management, care coordination, delivery of nursing interventions, case review, advanced care planning and risk management to the Continuing Care Team.This role will also contribute to the planning, development, implementation, maintenance and evaluation of a broad range of treatment and prevention activities to promote recovery from mental illness.About the Team The Continuing Care Team (CCT) works individually and in group settings to provide ongoing mental health interventions. The service objectives are:
