Clinical Nurse (Cognition), Queensland Health This is a short-term vacancy for a registered Clinical Nurse until 11 January 2025 in the Community Acute and Non-Acute Services!

About us: Community Acute and Non-Acute Services is a multidisciplinary team that focuses on evidence-based practice, continuous quality improvement, risk management, including information-based decision making as mechanisms for improving the quality, standards and safety of the health service provided. CANAS aims to provide the best patient care possible, at the appropriate time in the appropriate place. The principal objective of CANAS is the substitution and avoidance of hospital presentation, admission and facilitation of early discharge to enable restoration of functional capacity.
About the role: The Cognition Service supports care for patients experiencing cognitive impairment (delirium and/or dementia) across acute care, aged care and community settings in Rockhampton and surrounding areas. Outreach assessment, education, or support (e.g., in-person, telephone, or videoconference consultation) is provided on request to other areas of CQHHS. Cognition Service is part of a group of services operating under a hospital avoidance model, called Community Acute and Non-Acute Services. As such, is it an ambulatory, transitional service aimed to assist the patient throughout their care journey.
About you: Mandatory qualification/s: Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).
Non-mandatory qualifications/requirements: Experience in education development and delivery would be highly desirable, but not essential.
Possess advanced clinical knowledge and skills in aged care nursing or gerontic, with a specific focus in evidence-based cognitive and dementia care considerations. High-level interpersonal and communication skills including negotiation, consultation, liaison, collaboration, and conflict resolution skills to enable effective relationships.
