Clinical Nurse (Case Manager) (Bundaberg) Queensland Health (Organisation site ) Integrated Care Rehabilitation Services, Bundaberg Hospital An exciting opportunity is currently available for an experienced Clinical Nurse (Case Manager) to join the Integrated Care Rehabilitation Team at the Bundaberg Hospital within the Wide Bay Hospital and Health Service. The Transition Care Program is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria. The position of clinical nurse (Case Manager) will have responsibility for the assessing, care planning, provision of nursing care, coordinating and monitoring of allocated clients within the Transition Care Program.
