Our focus is on respect, client-centred care, and harm prevention so people can lead longer lives, have improved health, meaningful employment, and a greater sense of belonging, safety, and security through our holistic model of care.

A bit about the role
We are seeking an experienced and strategic Clinical Lead to oversee our After-Hours Health Outreach Service (AHOS), which provides critical health interventions to adults with complex needs in Melbourne's CBD.
This role manages AHOS, our street-based outreach nursing program (operating Wednesday to Sunday nights) and non-medical clinical staff based at The Living Room, Melbourne's renowned Primary Health Service for people experiencing homelessness.
The Clinical Lead will drive the delivery of high-quality, client-centred care while enhancing access to multidisciplinary health services during after-hours periods.
A key responsibility of this role is to strengthen the systems and processes that support coordinated care between clinic and outreach-based services.
You will provide clinical leadership, coordinate daily operations, and ensure the professional development and clinical supervision of the nursing team.
Additionally, this role will take a lead in clinical governance, foster collaborative partnerships with external health, outreach, and housing services, and guide service integration within a multidisciplinary framework.

Key Requirements:
Demonstrated experience (2+ years) in a community health setting.
Demonstrated experience (2+ years) in delivering clinical interventions to disadvantaged populations.
Registered Nurse with AHPRA Registration
Experience in developing partnerships that deliver improved referral pathways and outcomes for clients
Demonstrated experience in leading a team of client focused, resilient and high performing staff
