As the Care Coordinator, you will be responsible for providing advanced clinical support, leadership, and coordination of clinical care for residents/clients with complex needs.
In this role, you will contribute as an integral member of the Seniors Living group and support activities aligned with the strategic direction of and consistent with the mission and values of Churches of Christ.
Your responsibilities will include: Overseeing and coordinating the completion of baseline and required focus assessments of individual residents/clients' needs;Ensuring that care plans are developed and reviewed based on the assessment findings of individual residents/clients' needs;Coordinating the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;Monitoring that resident/client documentation is accurately reflective of care provisions;Coordinating the care provision of external service providers;Assisting in the review and maintenance of clinical programs and initiatives;Coordinating the collection of clinical indicator data in accordance with the organisation's reporting timelines.The role offers the following availability: workdays are Monday to Friday, with every second weekend required. Flexible start time is either 7am or 8am.
