Location: We are seeking a Clinical Coordinator - Community Nurse Liaison who in conjunction with the Inter-professional Team coordinates the day to day patient care activities of the Ward/Unit, including identifying and implementing processes for admission, transfers, and discharge of Ward/Unit patients.
Launceston Community Nursing Service: Is a dynamic service that provides care to clients in the Launceston area in their home and community settings Nursing care provided is based on individual needs and the bulk of client presentations are for wound, palliative and continence care. Provides a wellness and reablement focus on care provision to enable clients to stay in the community and experience optimal quality of life The Community Nurse Liaison is a position that requires strong communication skills as they are the representative of Launceston Community Nursing who interacts with acute services such as the LGH to assist in discharge planning for safe client care.
The role: In conjunction with the Inter-professional Team coordinates the day to day patient care activities of the Ward/Unit, including identifying and implementing processes for admission, transfers, and discharge of Ward/Unit patients. Details of Appointment Permanent, full time, day worker position working up to 76hours per fortnight commencing from 4 August 2024

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies. Salary:$103,466- $109,165 per annum.
Our Employer 11% superannuation contribution is on top of this amount.
You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
In addition, the following allowances calculated on the salaried incremental point may be available:
Post Graduate Allowance of up to 7.5% based on qualification type Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023 and Nurses and Midwives (Tasmanian State Service) Interim Agreement 2013 Successful applicants will be required to meet the essential criteria:
Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse *Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee's responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered. Applicants should note the following criteria are desirable:
