This is a fantastic opportunity to join our dynamic team as a Clinical Coordinator at Bene Home Care. You will play a vital role to co-design, refer and coordinate the delivery of services to clients with complex care needs. This is a key leadership position within our team as we continue to grow our Home Care services.
What will you be doing? Conduct comprehensive assessments and reviews of client goals, needs, and preferences.Develop complex care and behavioral support plans in collaboration with Case Managers.Partner with Case Managers, clients and families to support identified care needs.Identify and refer clients to internal or external providers, as necessary.Provide education programs related to clinical assessment and practice, including medication credentialing.Stay up to date with best practice guidelines and provide clinical supervision to Registered Nurses as required.Lead clinical quality improvement initiatives across Home Care, including participating in clinical audits.Provide guidance and support to clients and families navigating the aged care journey.Contribute to the oversight and investigation of high-risk client incident reports.What are we looking for?
