At LDK Seniors' Living, we are seeking Clinical Case Managers to join our high-performing team! Our ideal candidate would enjoy administrative functions, as this role is predominantly non-clinical. Working from Monday to Friday, our Clinical Case Managers' key responsibilities include:
Assessment and Care Planning:

Manage care for residents, ensuring their physical, emotional, and social needs are met.
Implement personalised care plans in collaboration with residents, their families, and other health professionals.
Maintain records of care plans and government-funded home care packages, health changes, and any interventions provided.

Resident Engagement:

Coordinate with key stakeholders in the organisation to address resident care needs.
Determine eligibility and manage residents' entitlement to government-funded care.
Collaborate with the lifestyle team to develop social, community service and activity programs to meet each resident's goals and foster community connectedness.

Stakeholder Engagement:

Work closely with doctors and allied health providers to ensure ongoing care for our residents.

To be successful in this position, strong people skills are a necessity. Hands-on clinical duties are relatively infrequent, but you will need to be a very capable communicator.

Our successful RNs have come from backgrounds such as:

Home Care Package Management
Community Care Sector
General Practice
Geriatric Wards (with administrative exposure – discharge forms etc.)
Mental Health Nursing (with administrative/case management duties)
Dementia Care

We will provide full training to ensure the successful candidate can complete the administrative functions of their role successfully and build relationships.
