The RoleWe are looking for a Clinical Case Manager in the Home Care team, who is passionate about a client-centered approach to care.
The Case Manager ensures safe, high-quality care for home care clients through direct case management and the provision of advice and support to the home care workers.
The Case Manager provides a range of support and coordination services aimed at the provision of safe, high-quality, consumer-directed home care.
You must be dedicated to working in the Home Care Model and be able to apply applicable quality standards, including the Aged Care Quality Standards, NDIS Practice Standards, and Attendant Care Standards.
What you will doProvide case management and support using knowledge of the home care funding streams (e.g.
Home Care Packages Program, NDIS Disability Service, Home Care Packages, DVA Community Nursing, Other funding streams that require complex care)Promote and ensure Care Plans are developed in true partnership with clients.Support the investigation and resolution of complaints related to care.Provide coaching and mentoring for carers in relation to care and career development.Who are you and what will you bring?1 year of experience as a Case Manager in a home care settingMust be a Registered Nurse with a current valid AHPRA Registration.Be passionate about delivering a person-centred approach to careStrong verbal and written communication skillsAbility to work autonomouslyHold a current NDIS Worker Check (or willing to obtain)Current First Aid CertificateCurrent NSW Drivers license + comprehensive motor vehicle insuranceInterested?
