Clinical Case Manager - Empowering Older Adults to Thrive. Are you passionate about helping older adults live their best lives? Do you thrive on making a difference and guiding individuals toward improved well-being? Do you excel at time management, working systematically, and taking ownership of tasks?
As a Clinical Case Manager, you will play a pivotal role in our 8-week Short Term Restorative Care Program. The role is to provide care management and coordination, not care delivery. Administration tasks are no match for your meticulous attention to detail and knack for keeping things organised.
Responsibilities:Partner with clients to achieve wellness outcomes in Canberra.Conduct Clinical Case Management and Health assessments.Educate clients, carers, and their families around wellness and reablement.Facilitate group workshops and support sessions focused on promoting healthy lifestyle and self-management skills.Coordinate care with multiple disciplines including GPs, specialists, and our Allied Health team.Empower and support clients to achieve their optimal levels of functioning independence and self-care.Build and maintain relationships with local health professionals and services.What you need to be considered for the role:Degree qualification in nursing or allied health.Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA).3+ years of clinical or health management experience in Nursing or Allied Health.Experience within a health or community aged care setting.Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion.Strong interpersonal skills, problem-solving, analytical and reporting skills.Current driver's license and ability to travel within the region on a regular basis.If you have a passion for supporting the health and well-being of older adults, we would love to hear from you!
