Headquartered on the northwest coast of Tasmania, our philosophy is: Everyone deserves the right to a well-lived life, with dignity and respect.The Position:The Clinical Case Manager is responsible for managing a cohort of Home Care Package Clients including:Manage client care plans, ensuring all services and resources are utilized to enhance client health outcomes.Conduct clinical assessments and provide hands-on care, along with clinical guidance for other team members.Serve as the primary contact for clients, families and advocates.Lead the onboarding process for new clients.Oversee planning, implementation and ongoing evaluation of care.Ensure all client services are delivered in line with Respect Aged Care's values, organizational policies, compliance standards, and budget requirements.This role offers flexibility and autonomy, based at the Lindisfarne office with opportunities to travel within the local areas.Applications will be reviewed and assessed on a rolling basis as they are received.
