What will we offer you: A supportive team and positive work cultureFlexibility, work / life balance and variety in your working weekSpecialised employee benefits program and savings to reduce the impact of cost of livingOpportunities to grow your careerAs a Clinical Care Manager, you will: Deliver excellent client care for elderly AustraliansUse your experience, customer service and aged care management skills to make a mark on the community aged care sectorManage Homecare package programs in conjunction with overseeing individualised financial budgets to maximise the care for each clientEngage with internal teams to facilitate new client on-boarding through the lead to conversion processDevelop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionals to deliver on each client's specific needsManage and oversee a high standard of care provided by the myHomecare Group providers and staffInfluence and partner with our schedulers, clinical team, support staff and externalWork in collaboration, with the Regional Manager to develop and shape our service offeringsEnsure compliance with all relevant legislative and industry standardsProactively engage in continuous improvement across practice and service deliveryYou will have: Experience in the aged care sector preferably a community settingKnowledge of Home Care Packages, Commonwealth Home Support Program and Aged Care Quality StandardsDemonstrated ability of managing a diverse portfolio of clients with a range of complexitiesThe ability to travel as this role will entail face to face client meetings along with an office base of Clayton VICProcura program experience is desiredTo be successful you will need: The right to work in AustraliaCurrent First Aid and CPR certificatesAustralian drivers' licence, vehicle registration and access to your own carNational Police Clearance or willingness to obtain (myHomecare will organise via WorkPro at no cost to you)Registered Nurse qualifications with an unrestricted AHRPA registrationIf this sounds like something you want to be part of apply today
