Clinical Care Coordinator-Rn

Our Melbourne North office has recently started and is looking for an experienced and dedicated Clinical Care Coordinator to support our growth.
The successful candidate will preferably have a strong background in community aged/disability care. The Care Manager role will require coordination and delivery, including ongoing client assessments and the development of care plans that deliver flexible and personalized in-home care support.

QualificationsBachelor of NursingCurrent APHRA RegistrationCurrent First Aid & CPRDriver's LicenseNational Police CheckPreferred SkillsExperience in providing direct client care and ensuring safety.Good understanding of the government funding environment for aged care and NDIS (not compulsory but preferable).Good understanding of the hospital discharge environment.Good understanding of health assessments and developing care plans to meet clients' care needs.Ability to multitask and manage time effectively.Excellent verbal and written communication skills.Intermediate to advanced computer skills.
