Clinical Care Coordinator/Care Manager

Currently seeking a CCC/CM to join standalone NFP organisation in a small community feeling town in TAS. 

Described by the manager as the best structure and ran facility she has worked in, you will work with a fantastic team of staff to run the clinical operations in the home. 

Ideally looking for a strong CCC/CM to join the team but due to the experience of the current DON, this facility are open to training a strong RN with the right attitude into the role.
