About ECHO
ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.
We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP.
Our goal is to provide services at the same time, on the same day and with the same team of Support Workers to ensure continuity of care and trust.
Job DescriptionThe role: The Clinical Care Coordinator will bring a clinical focus to our person-centred care model, supporting elderly clients to achieve their goals and maintain independence.
This role includes acting as a liaison with clients, families, caregivers, and health professionals while offering clinical insights and support to the team.
Your responsibilities may include:
Managing a caseload of home care packages, complex, and high needs clientsEnsuring services meet internal and external quality standardsDriving innovation and continuous improvement aligned with ECHO's strategic objectivesOffering guidance and support to clients, families, and team members to deliver safe, effective careHours: Monday to Friday, 8:00 am to 4:00 pm.
You will also be required to participate in a rotating on-call after hours, weekday and weekend roster.
