Nursing - Aged Care (Healthcare & Medical)
As the Clinical Care Coordinator, you will coordinate and lead the care team in consultation with the Facility Manager. You will ensure compliance with legislative and statutory requirements to enable us to continue to deliver a high standard of care and most importantly, a quality lifestyle for our residents.
Working within a supportive and dynamic team, you will be provided with a challenging and diverse work environment, as well as an opportunity for you to contribute and make a real difference to the residential aged care community.
This is a part-time role to cover the CCC shifts on Thursday and Friday (8.00 AM - 4.00 PM).
Duties & ResponsibilitiesManage all clinical services in line with Regis process and policyAdvocate the implementation of continuous improvement activities in all aspects of clinical and lifestyle servicesEnsure all clinical documentation is completed accurately and on timeCoordinate and support all scheduled audits and clinical indicators completed in line with the scheduleEnsure resident health records and clinical documentation are relevant and up to dateEnsure "best practice" medication management guidelines are observed and regularly monitoredAbout YouAs a collaborative and senior member of the operations leadership team, you will possess:
Relevant tertiary qualifications or Australian recognised equivalencyCurrent registration with AHPRA as a registered nursePrevious senior clinical care experience in the health sector with clinical experience in aged careSound understanding of care assessment in the aged care sector and use of AN-ACCDemonstrated effective interpersonal and communication skills (written and verbal)Demonstrated proficiency in the use of a wide range of IT applicationsGood working knowledge and understanding of AN-ACC funding and aged care standardsTo work in residential aged care, you will need to provide satisfactory background checks, supply an NDIS worker screening clearance, COVID and flu vaccinations, and certification proof.
