Nursing - Aged Care (Healthcare & Medical) Location: Lismore, NSW
Hours: Monday to Friday, 38 hours per week (no weekends or evenings)
Status: Permanent Full-time
Are you a dedicated Registered Nurse with a passion for leading clinical care in aged care? We are seeking an experienced Clinical Care Coordinator to join our client's team in Lismore. This is a pivotal, full-time role responsible for overseeing high-quality clinical care, emphasising safe, effective, and person-centered care tailored to each client's unique needs.
About the Role: As the Clinical Care Coordinator, you will work closely with the Aged Care Manager and clinical team to ensure the physical, mental, and emotional well-being of clients. Your leadership will inspire excellence within the team, support continuous quality improvement, and maintain our client's commitment to compassionate, client-centered care. In this role, you will conduct in-home clinical assessments, offer clinical support, and manage complex care needs.
Key Responsibilities: Clinical Leadership and Oversight: Lead the clinical care team, ensuring best-practice care that promotes client health and well-being. Support and supervise Registered Nurses, Enrolled Nurses, and other clinical staff, fostering consistent care standards. Alignment with Aged Care Standard 3: Ensure clinical care is safe, effective, and respectful, meeting clients' goals and preferences. Manage clinical risks and address any health changes promptly. Risk Management and Quality Improvement: Identify and address improvement areas, participate in audits, and ensure compliance with Aged Care Standards. Make timely referrals to external providers when needed. Staff Development and Performance Management: Mentor and support clinical staff, promoting their professional growth through training, performance feedback, and competency assessment. Clinical Care and Service Coordination: Conduct comprehensive assessments and coordinate clinical handovers and case conferences, ensuring seamless care transitions. Selection Criteria: Registered Nurse (RN) with AHPRA registration. Extensive experience in aged care with a strong understanding of Aged Care Standard 3 and clinical compliance. Proven ability to lead a clinical team with empathy and resilience. Strong organisational skills and the ability to manage priorities. Excellent communication and assessment/report writing skills.
