The position ofClinical Care Coordinatorentails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members and/or their families.
The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at-risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.
The role requires you to: Liaise and form partnerships with various internal and external stakeholdersDirect provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.Maintain clinical records, collating and analysing data from client records & preparing reports as requiredWork systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and familiesParticipate in community development and service promotion activitiesContribute to the development and delivery of group programsAbout you: Degree qualified Registered Mental Health Nurse, Psychologist, Social Worker or Mental Health Occupational TherapistFull registration with AHPRA or AASW (eligible for MH Accreditation)Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card)AnAustralian Citizenand willing to undertake a National Police Clearance.In return you will receive: An attractive hourly rate of$65 per hour + super12 monthsMonday to Friday, 37.5 hours per weekHybrid model (2 days from home)Extensive training and ongoing support& developmentTo express your interest in this position please submit your application otherwise please contact Mel Ireland, ****** for a confidential discussion.
