Apply focussed people & data processing skills as the vital conduit between mental health care specialists in Emerald & staff in regional Community Health locations
This role is based in Emerald Make a difference every day!
Work with the management team to establish and deliver Clinical Care Coordination services for Emerald.
Long established and trustworthy, this community-based support organisation has a well-earned reputation for providing mental health and suicide prevention services to people and their families.
This organisation is focused on improving the wellbeing and quality of life outcomes of people across the community, utilising a person-centred approach to care.
Through its accessible person-led services, it helps individuals across the mental health spectrum achieve more independent and positive 'lives beyond illness'.
If you're passionate about working with complex and severe mental health issues and enjoy working in a supportive environment, this could be an exciting opportunity for you.
As a Clinical Care Coordinator, you're a pivotal part of a healthcare orchestra—collaborating to ensure that all the instruments (or in this case, multidisciplinary mental healthcare professionals) play in harmony to create a beautiful symphony of client care.
In this intricate dance of mental health care, your pen waltzes across care plans, stitching hope into every line.
You're the bridge between client, families, and the entire healthcare ensemble.
Your inbox is a bustling marketplace of emails, voicemails, and carrier pigeons (okay, maybe not pigeons).
You ensure everyone's on the same sheet of music.
You're part therapist, part cheerleader.
Clients and families lean on you during tough times.
Your empathy is a soothing balm.
Your work isn't just about charts and checklists, you weave compassion into the very fabric of healthcare.
Like a gardener tending to delicate blooms, you monitor client progress.
If the treatment plan needs tweaking, you adjust it.
Flexibility is your superpower.
This is a clinical position.
Candidates must have a tertiary qualification or equivalent in Mental Health or a related discipline.
Experience or knowledge of the Mental Health, Alcohol and Other Drugs (AOD) and Suicide Prevention sector and an understanding of client needs are essential.
A minimum of three years' experience is required.
Accredited Mental Health Clinicians (such as Psychologists, Social Workers or Mental Health Nurses) are preferred due to their expertise in providing therapeutic support.
AHPRA registration or membership with AASW is essential.
Organised and unflappable, with a complete understanding of patient confidentiality and a strong sense of social justice, your enquiring mind will see you fully engaged with this worthy mission to contribute to positive client outcomes.
Always up for a challenge, you will gladly take responsibility to ensure that the part you play in this important initiative runs perfectly smoothly and consistently error-free every day.
