Clinical Care CoordinatorAdelaide CBD - Full TimeProgress with purpose at one of Australia's largest health care providersGet ahead with salary packaging, benefits and professional development in a supportive team environment.About the roleBased in Adelaide CBD, we are looking for a full-time Clinical Care Coordinator who will be responsible for coordinating clinical services in order to support clients in their own home.Working as a team, you will be the primary point of contact in supporting clients and staff to ensure services are delivered efficiently and provide clients with the best possible outcome.Your main duties will include:Daily coordination of care for new and existing clients of the SA Health Community Care Program.Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available to commence service delivery within the required service delivery timeframes.Identify clients in the clinical programs that are eligible for other Government funding streams and liaison with the referrer, MRU or healthcare professional for transfer of care.About youBachelor of NursingExperience in Acute NursingCommunity nursing experienceFull Drivers LicencePrevious experience managing service contracts.Previous experience managing staff.Strong problem-solving and time management skills.Effective communication skillsComputer skillsAbility to work over a 7-day rosterLogistical experience (desirable)Why work for Calvary?At Calvary, our staff matter.
