Clinical Care Co-ordinator Sydney Northern BeachesLocation: Sydney Northern Beaches, New South WalesAn opportunity has arisen at Right at Home Sydney Northern Beaches for an energetic self-starter, who is a highly motivated and experienced individual, to join our business as a Clinical Care Coordinator (CCC) on a Full-Time basis.
We are seeking to recruit dedicated individuals who strive for best practice, are able to communicate well with people of all backgrounds, and approach every situation with care and compassion.

The Care Coordinator will be responsible for:
Building strong connections with clients and their familiesAssessing client needs and resources for health, welfare, and other facilities and servicesConducting Client Care Assessments and Care PlanningMaintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkSupporting all clients to meet their goalsSupporting and providing feedback to Home Care WorkersRe-organising work priorities to meet the needs of a fast-paced environmentPromoting the values of the company and planning supports using a contemporary approachQualificationsThe successful candidate will have:
A current registration certificate for a Registered Nurse.Excellent skills in assessing, documenting, and implementing client's care planning objectives.Experience in assessing and planning care needsSound understanding of the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding FrameworkOrganisational, administrative, and time management skillsAbility to work in a team setting, both as a participant and a leader.Well-developed problem-solving, interpersonal, written, and communication skills.All candidates must have the right to work in AustraliaAbility to Obtain:
National Police CheckDrivers LicenceNDIS Worker Screening CheckFirst Aid & CPR CertificateJob Type: Full-time
