As a Clinical Care Assessor, you'll bring your professional expertise to the forefront, shaping the care of older Australians by providing clinical guidance, managing complex cases, and ensuring every individual feels supported, respected, and valued.

As part of the Australian Government's new Single Assessment System, we're growing our team over the next six months and inviting skilled clinicians to join us.

What You'll Do: Conduct in-depth assessments for older Australians, incorporating clinical expertise to address complex needs and situations.Collaborate with a multi-disciplinary team of nurses, allied health professionals, and medical experts to ensure the best outcomes for clients.Identify when additional specialist interventions, reablement strategies, or advanced care planning are required and provide appropriate clinical recommendations.Develop tailored support plans that empower clients to achieve their best quality of life.Provide guidance to match clients with the aged care services they need, whether in-home or residential.Be part of an inspiring team leading change in the way aged care services are delivered.Why This Role Matters: You'll be the first point of contact for people seeking help, and your work will shape the care and support they receive.
You'll provide the clinical expertise needed to navigate complex care needs, ensuring that every assessment results in the best possible outcomes.
This role goes beyond standard assessments—it's about advocacy, empathy, and being a vital part of Australia's broader mission to improve aged care.
What You Bring: Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker, with current AHPRA registration (or equivalent registration authority).Experience in aged care, health, or community services, particularly in complex case management.Great communication and collaboration skills—you'll work closely with clients, families, and a multi-disciplinary team to achieve the best outcomes.A strong understanding of cultural competency—you value diversity and adapt your approach to meet the needs of people from all backgrounds.Confidence in managing clinical processes, systems, and decision-making to support advanced care planning.A passion for helping others and making every interaction meaningful.What's in It for You?
