We will be offering contract positions, in line with government funding.
We have multiple positions available within our Assessment Service teams throughout Canberra and the surrounding area.
COA is open to offering full-time and part-time positions to the right applicants.
You will be reporting to the Assessment Team Leader but must be able to work autonomously.
The role responsibilities include:
Completing face-to-face assessments to determine older Australians' eligibility for support within My Aged Care.Reviewing previously assessed clients.Undertaking client-centred support planning using a wellness and reablement approach.Liaising with referral sources and community support organisations to assist our clients in obtaining the home supports they require.Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.Preferred candidates will have: Allied health or Social Work tertiary qualifications.Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of a relevant professional association.Ability to work independently and as a team member.High level of computer literacy skills.Ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity.Current Australian driver's license with own reliable, fully insured vehicle (Vehicle Allowance applicable under SCHADS Award).Willingness to complete face-to-face home visits within their region.Living within the Canberra & surrounding region.Valid National Criminal History Check and Working with Children Check (NSW or equivalent).Vaccination (COVID & Flu) or willingness to update to industry requirements.COA is committed to being an equal-opportunity employer and encourages applicants of all cultures & diversities.
