Reporting to the Client Services Manager, this is a dynamic role that acts as a key bridge with clients ensuring the right information is available to our fantastic support crew for quality outcomes.Key Accountabilities include:In consultation with the Client Services Manager facilitate quality client service delivery for our diverse client base located across Southeast Queensland.Understand client support needs / goals and briefing the same to relevant stakeholders for each new intake.Provide client service information to rosters and recruitment teams to ensure optimum support worker matching.Provide client support in an accurate and timely fashion including keeping client files up to date and responding quickly to situations as they arise.In collaboration with the People and Culture team, ensure support workers are briefed on client specific health and welfare requirements eg seizure, medication, manual handling needs and records updated.Support program / activity design and planning as delegated by Manager including resourcing and occasional in-person support.Close follow up communication with clients and carers as needed.Prepare NDIS Progress Reports for plan reviews with Intake and Quoting team.Provide Compliance team with documents and assistance as required for Incident and Feedback management.Participate in the Client Services On-Call Roster.This position is aligned with a Level 3 classification in the Social Community Home Care and Disability Services Award.Benefits of joining FocalGenerous Salary Packaging benefits up to $15,900.Learning and Development opportunities.Access to our Employee Assistance Program.Supportive People and Culture team.About youReliable | Motivated | Passion for positive outcomesTo be eligible for this role you would have at least 2 years' experience in client coordination, preferably in the disability sector with a relevant qualification in community services or a willingness to work towards one.Excellent administration skills and comfortable with using digital tools.Good decision-making skills ensuring effective supports and incident response.Excellent attention to detail.Valid Australian Driver's Licence along with a comprehensively insured vehicle.Current First Aid & CPR Certificate.Blue Card & Yellow / NDIS Worker Screening Clearance Card.Strong work ethic and good reliability.If this sounds like you, please apply today.Role Type
