About the Role Based in Ipswich, we are looking for an experienced professional who is able to hit the ground running. Reporting to the Client Services Manager, this is a dynamic role that acts as a key bridge with clients ensuring the right information is available to our fantastic support crew for quality outcomes. Key Accountabilities include: In consultation with the Client Services Manager facilitate quality client service delivery for our diverse client base located across Southeast Queensland. Understanding client support needs and goals and providing briefing to relevant stakeholders when new client commences Provide client service information to rosters and recruitment teams to ensure optimum support worker matching Undertaking client file recording and reviews to ensure information is up to date and accurate using client management system and electronic client files. In collaboration with the People and Culture team ensure support workers are briefed on client specific health and welfare requirements eg seizure, medication, manual handling needs and records updated Support program and activity planning as delegated by Manager including resourcing Follow up communication with clients, families and carers as needed Prepare NDIS Progress Reports for plan reviews with Intake and Quoting team Provide Compliance team with documents and assistance as required for Incident and Feedback management Participate in the Client Services On-Call Roster This position is aligned with a Level 3 classification in the Social Community Home Care and Disability Services Award. Good decision-making skills ensuring effective supports and incident response.
