We are looking for a person who is experienced in implementing and monitoring all aspects of a person's NDIS plan. Your role will be to support our clients who currently live independently in their own home with complex care needs. You will be personable, collaborative and self-motivated. Knowledge of the NDIS environment is essential, with a genuine desire to support people with disabilities to live their lives as they wish. Frequent travel within the outer south east of Melbourne would be required. If this sounds like you, then we would love to hear from you!
The Organisation Experience Life is an incorporated not-for-profit organisation providing supports for people with disabilities across Melbourne. We work directly with the individuals and their families to assist people with disabilities to achieve their goals. We empower our clients by ensuring they are supported and are directly involved in making decisions that impact their lives. We work to create personal plans focused specifically on them and their needs.
The fundamental belief that all people with disabilities have the right to actively and meaningfully participate as members of mainstream society helps guide our approach and services. The Role Reporting to the CEO, the client services role will focus on the day-to-day quality of care provided by our support workers in clients' homes. They will also participate in initiatives to enrich our positive workplace culture and ensure we are in a strong position to continue to provide person-centred supports in a changing regulatory environment. This home-based role will enable the right candidate to work autonomously while getting the job done. Some out of hours flexibility would be required.
The key responsibilities of the role include, but are not limited to:
Providing guidance in the implementation and monitoring of outcomes and daily support to our clients Responsible for support of direct care staff in conjunction with the People and Culture Manager Ensuring compliance with NDIS and broader legislative requirements About you We are seeking someone who has:
Qualifications in the Community Services area, with sound experience in case management A demonstrated ability to develop positive relationships with clients and staff A sound understanding and practical working knowledge of providing disability care services in clients' homes where complex and high-care needs are involved The ability to supervise and support a remote team of staff A comprehensive understanding of the NDIS and the compliance framework Exceptional interpersonal skills and the demonstrated ability to interact with a diverse range of people, including those with physical and cognitive disabilities Own vehicle and current Victorian driver's licence (essential as this role will involve frequent travel within outer south east Melbourne) APO experience is desirable or familiarity with behaviour support plans The successful applicant must provide or be willing to obtain:
