As our Client Service Coordinator, you'll support NDIS clients and their families, crafting personalised support plans and ensuring top-notch service delivery. It's all about empowering clients and making every day better.Key Responsibilities:Empower Clients: Coordinate and manage supports to enhance clients' quality of life.Personalised Plans: Work with clients and families to deliver person-centred support in line with NDIS plan goals.Support Leadership: Guide support workers for effective plan implementation.Efficient Rostering: Assist the rostering team to ensure smooth service delivery.Continuous Improvement: Participate in team meetings, problem-solving, and enhancing services.Data Management: Maintain client data integrity using Visual and SharePoint.Community Relations: Build strong relationships with stakeholders and enhance Allcare's community reputation.Specific Duties:Manage Supported Independent Living (SIL) properties and participant accommodations.Conduct initial assessments and develop care plans for clients with disabilities.Coordinate access to services, advocate for clients, and manage cases.Provide guidance to families and caregivers, maintain records, and respond to crises.Ensure compliance with quality standards and regulatory requirements.Key Skills and Experience:Integrity, diversity, and respect in professional practice.Experience in legislative, funding, and quality management frameworks.Strong communication and teamwork skills.Proficiency in Microsoft Office and industry-specific software.Ability to manage time effectively and work under pressure.Understanding of Disability Framework policies and procedures.Commitment to Allcare's mission, vision, and values.Valid driver's licence & car.NDIS Worker Screening clearance.NDIS Worker Orientation Module completion.Working with Children Check (DCSI).Certificate III in Disability Services or equivalent.Current CPR Certificate (if not covered in Senior First Aid Certificate).Police check valid within 12 months.How to Apply: To apply, submit your resume and a cover letter outlining your suitability.
