Client Services Co-ordinator CareWays Community has been providing services to the community for more than 30 years through in-home care and support for the elderly, Neighbourhood Centres and Youth Services.
About the role We are looking for an experienced Client Services Co-ordinator with initiative, judgement, and administrative ability. This position is based out of our head office at the Dapto Ribbonwood Centre. It is a fulltime position, working Monday to Friday with an option of a monthly ADO. This role will also be required to participate in the oncall weekend rotation roster to provide support and guidance to the Care workers as required.
The Client Service Co-ordinator will enhance the quality of our client's experience whilst promoting their health, wellbeing and continuity of care. This position works collaboratively with clients and key stakeholders to provide individualised case management. The Client Service Co-ordinator plays a key role in promoting the client's goals and wellness outcomes and with collaboration build client numbers with the team.

Relevant tertiary qualifications at Degree/Associate Diploma/certificate 4 level (Business, Admin and/or aged care) and or 4 years' experience in a similar positionExperienced in case management within the home and community care sector with advanced knowledge of person-centred careAccurate data entry skills and computer literacy in MS OfficeDemonstrated experience in Home Care service delivery, procurement, and contract management.Experience in service development and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality standards.Highly developed interpersonal, communication skills and emotional intelligence with the proven ability to foster relationships and connect with internal and external stakeholders.Current drivers' licence and working with children's check.COVID Booster vaccinationHow to Apply Please send your application and resume, outlining your qualifications and experience to:
