About the role The Client Liaison Officer undertakes assessment, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals.
This role supports clients to manage and monitor their budget to ensure care packages are financially sustainable and monitors the services provided to ensure these are following relevant service models and guidelines.
This is a Team Leader position offering an opportunity for career advancement and responsibility.
Skills and Experience Experience with Aged Care programs highly regarded Proven understanding of assessments, packaged care and case management Ability to engage clients and work effectively with them to achieve identified goals Support and guidance to clients with budget management and financial sustainability Experience in monitoring and assessing services provided to ensure they are following service models and guidelines Exceptional communication skills – both written and verbal – and the ability to relate to a wide range of people Ability to advocate strongly for your clients and the services they require A sound understanding of the community care system and the people who utilise this service Diplomacy and negotiation skills; people management capabilities Along with a Certificate IV in Health, Community Services, Aged Care or Disability Support, current Driver Licence and a National Police Certificate.
We also highly value Qualifications relating to Case Management.
