Client Liaison - Roma

About the role As a Client Liaison Officer you will undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist them in achieving their identified goals.
Support the client to manage and monitor their budget to ensure care packages are financially sustainable and monitor the services provided to ensure these are following relevant service models and guidelines.
This role is based at our Roma Service.This is a Permanent Part-time position of 30.4 hours per week for a contract period of 7 months.What we are looking for: Certificate IV Ageing, Cert IV in Lifestyle & Health, Cert IV in Disability or equivalent Current National Police Certificate - or willingness to obtain Current Drivers Licence and own reliable vehicle To be successful you will also bring: Proven understanding of assessments, packaged care and case management Ability to engage clients and work effectively with them to achieve identified goals Support and guidance to clients with budget management and financial sustainability Experience in monitoring and assessing services provided to ensure they are following service models and guidelines Exceptional communication skills – both written and verbal – and the ability to relate to a wide range of people Ability to advocate strongly for your clients and the services they require A sound understanding of the community care system and the people who utilise this service Diplomacy and negotiation skills; people management capabilities Why Join Anglicare?
