About the role
Part time permanent (4 days/week) position based in Milton, QLD. Reporting directly to the Team Leader Client Services, you will be responsible for:
Supporting carers of people living with dementia via telephone service connecting clients with relevant service options Administering and completing registration, assessment and planning activities with carers who are accessing counselling and coaching services Providing a seamless experience for carers and their families by delivering personalised support, information and referral pathways Conducting risk assessments and developing action plans to mitigate risks Engaging clients in meaningful conversations where their needs are actively listened to, understood, and effectively addressed What is in it for you?
Salary benefits: We offer all employees the opportunity to salary package up to $15,900 each year for living expenses and up to $2,650 each year for entertainment benefits.

About you
To be successful in this role, we are looking for someone with the following:
A relevant qualification in a health-related field and/or significant experience working with clients in the delivery of community-based services. Experience in engaging and building rapport with clients 1:1 in a service delivery environment. Knowledge and understanding of the impacts of dementia for individuals, carers and their support networks. Knowledge of service systems available to meet client needs, including My Aged Care and the Carer Gateway. Ability to build, foster and sustain positive relationships internally and externally.
