Reporting to the Business Improvement Specialist, you will be the first point of contact for our Home and Community enquiries, playing a critical role in supporting clients to achieve their goals through access to Endeavour Foundation services.You will analyse disability assessments, conduct screenings, and create tailored support plans to ensure clients are matched with the right services based on their needs and funding. Additionally, you will play a key role in developing and implementing initiatives and activities that drive growth, boost customer retention, build community awareness, and create pathways for people with disabilities to access Endeavour Foundation services across Qld, Nsw, and Vic.In this varied role, you will:Understand and assess a client's support needs, confirming functional capability and ensuring staff and leaders have appropriate training to ensure specific support needs are met.Support a culture of person-centred practice which puts clients, families, carers, and guardians at the forefront of everything we do.Provide a customer experience in line with Endeavour Foundation's Values and behaviours, managing day-to-day relationships with clients, their families, and external referral organisations.Foster collaboration with regional site leadership teams, supporting the co-creating and co-delivery approach across and within the business.Through a comprehensive understanding of the NDIS and disability sector, match clients to life-changing services that support their goals and aspirations, maximising the usage of Individual NDIS funding levels.Assist in the development of a relevant pipeline of qualified leads aligned to drive the growth and sustainability of our core customer program, helping to identify new market opportunities.What you can bring to the teamWith a deep understanding of the NDIS and disability sector, you excel at building strong relationships and developing meaningful connections. Your ability to analyse and interpret complex disability assessments allows you to create comprehensive risk assessments, safety plans, and support strategies that meet individual client needs.You will also have:Degree in Social Work, Psychology, Human Services or a relevant field.Minimum 4 years' experience in the community services sector.Experience analysing support needs and creating comprehensive assessments to maximise NDIS funding opportunities.Large volume case management for disability or support networks is highly desirable.Excellent relationship management, interpersonal, and communication skills with an engaging and inspiring communication style to leverage further opportunities.Strong analytical skills with the ability to collect, analyse and disseminate data to drive customer conversion ratios and provide actionable insights for key stakeholders.Hold or be willing to obtain a Working with Children Check (Blue Card), NDIS Worker Screening Check (Yellow Card), National Criminal History Check (Fit2Work), and an open Driver's License with comprehensive insurance on your vehicle.Intrigued?
