Who are you? You come with a background in disability, mental health, nursing, social work, or an allied health discipline You understand community services and NDIS funding You have experience writing person centred support plans You have experience networking and creating long standing relationships with key stakeholders Ideally we are looking for someone who is comfortable visiting people in their own homes to discuss how we can assist them and assess any risks we may need to be aware of.
You will need to have the ability to juggle more than one thing at a time, this is a fast paced role and you will need to have an ability to prioritise and manage conflicting tasks and requests.
We are looking for someone with an understanding that living with a disability can be tough and genuinely wants to make a difference in client's lives. You need to be a good listener and communicator and have an empathetic attitude.
Key components of the role include: Building and maintaining a great network of referrers who will help you achieve your targets Meeting clients and their families and understand the supports they need Creating person-centred support plans that reflect the client's needs and aspirations Ensuring that clients and referrers have a positive experience Undertaking client risk assessments to keep everyone safe Heaps of 'feel-good' times.
