The Role:The Clinical Reporting RN (Care Planner) promotes safe, high-quality care and accurate recurrent funding for residents by ensuring care plans are accurate and up to date.
The Care Planner is responsible for assessment, care planning and care evaluation in accordance with company and resident policies and works closely with the Care Manager and Care Team to achieve their objectives.
What you will do:
Complete interim assessment and care planning on day of admissions Complete quality care evaluations Ensures care plans are developed in true partnership with residents Liaise with the company revenue and classification team (ACF1/AC ACC) to ensure accurate classification and optimise recurrent funding Provision of safe, high-quality care in accordance with the Aged Care Quality Standards Positive resident experience through resident-first approach Who are you and what will you bring?
Registration with the APHRA as a Registered Nurse is essential Minimum 2 year's experience in a care management role in the residential aged care setting Demonstrate strong verbal and written communication skills Hold and maintain NDIS Worker Checks as per NDIS Act 2013 (or willing to obtain) Be passionate about delivering a person-centred approach to care Come experience the charm, history, and warmth of Yackandandah —it's more than a place; it's an invitation to create your own remarkable story!
