Chsp Nurturing Lead (12 Month)

About the role: The CHSP Nurturing Lead plays a critical role in identifying and assisting elderly customers to transition from the Commonwealth Home Support Program (CHSP) to the Home Care Packages (HCP) program.
This involves a thorough assessment of future requirements, providing guidance and support throughout the process, and ensuring that the transition is as smooth and efficient as possible.
This role also educates customers and their families about the benefits and resources available through the HCP program, helps navigate any administrative hurdles, and assists in tailoring future funding packages to meet the specific needs and preferences of each customer.
Other responsibilities include: Source accurate pipeline data from My Aged Care Portal.
Use data to identify HCP allocation timeframes and report on successful HCP transitions.
Report on unsuccessful HCP transitions and identify barriers/themes.
Liaise with CHSP team to identify customers approved for HCP and may require ACAS assessments.
Develop action plans to manage positive customer engagement during the HCP assignment period.
Develop a strong working relationship with the sales team.
Collate and share monthly reporting to track performance, identify trends, and review customer experience.
About you: Comprehensive understanding of CHSP, HCP and relevant Aged Care legislation.
Excellent interpersonal and communication skills.
Strong report writing and documentation skills.
Advanced IT skills and ability to produce professional reports.
Ability to meet KPIs and manage performance effectively.
Experience in stakeholder negotiation and coordination.
Ability to work independently and proactively, identifying issues and potential solutions.
A genuine passion and interest in helping vulnerable elderly customers and their families.
Proven ability to influence positive outcomes for customers in line with business objectives.
