We are seeking a CHSP Funding Administrator.

We have been fortunate enough to gain a grant under the CHSP Growth Funding in the South Eastern Suburbs, and are looking for an experienced person to assist us in rolling this out, as well as helping with our application for My Aged Care.

The objectives of the CHSP program, which forms part of the Commonwealth-funded aged care system, include:
Providing high-quality home support services at a low intensity on a short-term or ongoing basis, or higher intensity services delivered on a short-term or episodic basis to eligible individuals, who are assessed as requiring such services to maximise their independence at home and in the community, enhancing their wellbeing and quality of life.
Supporting eligible individuals through linking them to appropriate and sustainable housing, community care and other support services.
Supporting eligible individuals through the delivery of planned respite activities which allow carers to take a break from their usual caring responsibilities.
Contributing to the delivery of quality and safe-funded aged care services in Australia.
This role will include, but not be limited to:
Working 1 day per week, hours are negotiable, both on and off-site.
Being required to attend occasional meetings face-to-face in and around the Frankston/Cranbourne area.
Occasional onsite visits with our customers.
Investigating and applying for additional grant opportunities within the disability and aged care sectors.
Coordinating the provision of home maintenance services that assist clients in maintaining their home in a safe and habitable condition, and assisting to maintain their independence, safety, accessibility, and health and well-being within the home environment.
Maintaining all financial recordings including performance reports, tax invoices, and all other compliance under the grant.
Collating and reporting on all wellness and reablement reports by the applicable due date.
All other reports as requested by the Commonwealth.
Collaborating with multidisciplinary teams, families, TIG staff and others to ensure holistic care and support.
Maintaining accurate and timely documentation and reporting anything as needed.
Adhering to all safety protocols and guidelines to guarantee a safe working environment for yourself and the people we support.
Must have or be able to gain asap:
NDIS Worker Screening Check.
Police Check.
Working with Children's Check.
First Aid and CPR certification.
Worker Orientation Module.
Worker NDIS Induction Module.
- Proven or lived experience with disability.

- Certificate III or IV qualifications in Aged Care, Disability, or a related field.

- Knowledge and understanding of disability support principles, policies, and practices.

The ability to communicate effectively with our customers, their families, and other stakeholders such as OT's, Physio's etc.
Strong interpersonal skills and the ability to build positive and professional relationships with the people we support.
The ability to work independently, as well as part of a team.
Excellent problem-solving skills and the ability to think on your feet in challenging situations.
