To Be Considered For This Position, You Will RequireCurrent registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience.Demonstrated experience in managing clients with chronic disease in particular chronic kidney disease and providing education in disease prevention and health promotion to clients, in particular brief interventions for risk behaviours and self-management.Knowledge of issues affecting the health and wellbeing of Aboriginal people in Central Australia.For More Information On The Position Please ContactTony Corcoran - Manager Care Coordination on or
